With the scores locked at 3-3 from a dramatic first leg in Spain six days ago, Inter made a fast start at San Siro in an attempt to go in front on the night and on aggregate. And Simone Inzaghi's side were rewarded for their impressive opening as Lautaro Martinez gave them the lead in the 21st minute.
That advantage was doubled just before half-time, as Hakan Calhanoglu hammered home from the penalty spot after Martinez was brought down inside the box.
At this point, it looked like Inter - who were 2-0 up on the night and 5-3 ahead on aggregate at the interval - were going to cruise through to the final. But it wasn't that simple.
Barcelona sparked a dramatic second-half fightback and scored THREE times to put them in front on the night and on aggregate. Eric Garcia pulled one back in the 54th minute with a neat finish, before Dani Olmo levelled on the hour mark with a thumping header.
Then in the 87th minute, Raphinha put Barca ahead for the first time in the tie as he fired home into the bottom corner at the second attempt after his first effort was saved.
Hansi Flick's Barca seemed in full control and Inter didn't look like they had any energy or desire left to get back into the game, up until veteran centre-back Francesco Acerdi popped up with a 93rd-minute equaliser. The 37-year-old was there inside the box to prod home from close range, sparking wild scenes inside the San Siro.
As the game drifted into extra time, the momentum shifted back to Inter and Inzaghi's team regained the lead on the night and on aggregate thanks to a fine finish from Davide Frattesi.
Barca pushed and pushed to get back into the tie and find a leveller, but Inter defended superbly in extra-time and managed to hold out for a historic win.
It means that Arsenal, if they beat PSG on Wednesday, will face Inter in the Champions League final in Munich on Saturday, May 31. The Gunners have already lost against the Italian side once this season, having suffered a 1-0 defeat in the league phase back in November.
